Participant Waiver, Release & Photo Authorization

Consent, Release and Authorization

I am the parent or guardian of the child(ren) registered for this program. I consent to the participation of Music Together® classes operated by Music Together Columbus, including but not limited to regular classes, demonstrations, trial classes, make-ups, sing-alongs, and pop-up/individual classes.

I understand that these programs involve physical activity and movement, and I assume all risk of injury or harm arising from participation. I release, indemnify and hold harmless Music Together Columbus, Elizabeth Cottle, Music Together LLC, and the owner and operator in which the applicable program is held, together with their respective directors, officers, employees, sponsors, and staff from any claim, suit, demand, or action arising out of or related to participation, including but not limited to claims of ordinary negligence, to the fullest extent permitted by law.

I further assume all responsibility for all transportation to and from class and events.

I HAVE CAREFULLY READ THE ABOVE CONSENT, RELEASE AND AUTHORIZATION AND FULLY UNDERSTAND ITS CONTENTS. I UNDERSTAND THIS IS A RELEASE OF LIABILITY AND I GIVE UP SUBSTANTIAL RIGHTS BY SUBMITTING THE REGISTRATION FORM ONLINE, AND I DO SO VOLUNTARILY.

Photo and Video Release

My child and I are participants in the Music Together® program. I hereby give permission to Music Together Columbus and Music Together LLC to take photographs and video recordings of my child, or me, or both of us together, and use them in printed, web, image, or video formats for advertising and educational purposes.

The images may be edited at the program's discretion, and I expressly release Music Together Columbus, Music Together LLC, and their agents, employees, and assigns from all claims which I may have for invasion of privacy, defamation, or any other cause of action arising from the exhibition or distribution of any materials in which my child and/or I appear.

If I wish to revoke this photo/video consent, I may do so by submitting written notice to Music Together Columbus, and revocation will apply prospectively.

General Terms

Governing Law: This agreement shall be governed by and construed under the laws of the State of Georgia, without regard to its conflict-of-law principles.

Severability: If any portion of this agreement is held invalid or unenforceable, the remaining provisions shall remain in full force and effect.

Entire Agreement: This document constitutes the entire understanding between parties and supersedes all prior discussion, representations, or agreements, whether written or oral, regarding participation in Music Together Columbus programs.

By submitting the registration form online and agreeing to this document, I represent and warrant that I am the parent or legal guardian of the child named in the registration and that we shall both be bound by this agreement.
